The form can be accessed through the above button.   If someone attending is under the age of 18, their form must also be notarized. Completed forms can be faxed to 321-622-6995 or emailed to info@houseoffreedom.org.  They must be received at the church office no later than 3:30pm, Friday, June 23, 2023.
